Purpose of This Form
This form may be used to request a re-score for situations such as:
- Adding or removing performance ratings
- Correcting a test code
- Correcting a site code
- Other scoring-related adjustments identified by the Site Coordinator
Multiple test takers can be included in a single request, and additional pages may be attached if necessary.
